Position Information
IMIRS INVESTIGATOR, REGION 3 (REGION 3)
Incident Management, Investigation and Response System (IMIRS) Investigator
PART TIME POSITION, 25 HOURS/WEEK.
Must be able to make home visits to consumer's in the Northeast part of Ohio.

Reports to: IMIRS Supervisor

Description: The investigator is responsible for the response to unusual occurrence reporting from consumer, case manager and or provider. Functions include reporting, notification and response, investigation, substantiating incidents, and maintaining records.

Qualifications: Registered Nurse, Licensed Social Worker, or Licensed Independent Social Worker.
Free from specified offenses as evidenced by a criminal background investigation.
24 months experience in Home and Community Based Services preferred.
Excellent communication skills both written and verbal.
Knowledge and experience involving home health care.
Knowledge of Medicare, Medicaid and community resources.
Familiarity with Incident Reporting Systems.
Demonstrate good judgement and integrity.
Ability to conduct an investigation objectively and without bias.
Excellent customer service attitude.
Demonstrated ability to prioritize tasks and organized work duties.
Computer literate.

Physical Requirements:
Ability to conduct home and facility visits of unknown environmental conditions, requiring climbing stairs, traversing uneven terrain and or gravel, and tolerating temperature extremes and inclement weather.

Responsibilities:
Understand and comply with CareStar policies and procedures.
Implement processes relative to comprehensive investigation of complaints, including reporting mechanisms, tracking functions, responses, and trending of data.
Maintenance of statistical information relative to the IMIRS department.
Reporting on a regular basis efforts and results of investigation to appropriate individuals as designated by Supervisor.
Assist in the preparation of a quarterly report and analysis of such information as required by the organization and the State of Ohio.
Assist in development of Policy and Procedure as delegated.
Participate in Quality Improvement activities as deemed necessary by Supervisor.
Attend all ODJFS/BHCS meeting and training sessions as deemed necessary by Supervisor.
Report all Level 1 occurrences to the IMIRS Supervisor within 24 hours of notification.
Other duties as determined by the Supervisor.
Adherence to CareStar Rule in performance of job responsibilities.
