Position Information
Case Manager (North of Dayton to Cincinnati and Southwestern Ohio)
Looking for case managers to provide services to persons affected by Multiple Sclerosis. Case manager will provide a comprehensive assessment, will assist in coordinating social, economic, and healthcare resources in an effort to assist clients in maintaining optimum physical health, emotional well-being and independence. Service area includes north of Dayton to Cincinnati and southwestern Ohio.

Qualified candidates will have knowledge of community resources, medicare and Medicaid. LSW, LPN/RN or related degree required.
